慢性化膿性中耳炎膽脂瘤應(yīng)該如何治療

一旦發(fā)現(xiàn)膽脂瘤，治療原則就是要根除病變組織，預(yù)防并發(fā)癥的出現(xiàn)。同時對于中耳傳音結(jié)構(gòu)被聽骨鏈破壞的患者，要采用手術(shù)的辦法去重建中耳傳音結(jié)構(gòu)。因此中耳膽脂瘤主要的治療方式，就是手術(shù)目的是徹底清除病變組織，包括膽脂瘤、肉芽以及病變的骨質(zhì)和黏膜等等。
然后是要盡量保存原有的聽力，術(shù)中要盡量保存健康的組織，尤其是和傳導(dǎo)聲音密切相關(guān)的中耳結(jié)構(gòu)，比如聽小骨殘余鼓膜、聽骨鏈等等，這樣為下一步做聽骨鏈重建打好基礎(chǔ)。切除中耳膽脂瘤，具體的手術(shù)方式可以有上鼓室開放術(shù)、完壁式手術(shù)或者是開放式乳突根治手術(shù)，還有改良的乳突根治手術(shù)。
術(shù)式的選擇應(yīng)該根據(jù)病變范圍、咽鼓管功能狀況、聽力受損類型程度，以及有沒有并發(fā)癥，還有乳突本身的發(fā)育情況而定。經(jīng)過正規(guī)的手術(shù)治療，絕大多數(shù)的患者都能夠獲得干耳的結(jié)果。
