肛周膿腫早期癥狀多數(shù)表現(xiàn)為肛門(mén)直腸的墜脹疼痛，基本上局限在一個(gè)地方，而且疼痛發(fā)展非常迅速，一兩天或者三五天就會(huì)影響坐臥，甚至行走，排便的時(shí)候疼痛癥狀會(huì)加重，尤其是大便比較干燥的時(shí)候。根據(jù)膿腫的部位，疼痛程度不一樣，深部早期可能癥狀并不明顯，或者只表現(xiàn)了肛門(mén)脹的感覺(jué)或者下墜的感覺(jué)，可能伴有發(fā)熱。
相反，如果膿腫比較表淺，基本上沒(méi)有明顯的全身癥狀，不至于發(fā)燒，但是疼得比較重，局部表現(xiàn)為隆起、摸得到的硬結(jié)或者硬塊，越來(lái)越大，質(zhì)地比較硬，一般5到7天以后慢慢的局部變軟，屬于成膿期，應(yīng)當(dāng)做切開(kāi)引流的手術(shù)。
