口腔扁平苔癬具有一定的惡變傾向，但是它的癌變率并不是很高，報(bào)告顯示是2‰到1‰左右。扁平苔蘚只有少部分會(huì)癌變，但是世界衛(wèi)生組織是把它歸為潛在惡性疾病，有可能癌變。一般反復(fù)糜爛的扁平苔蘚才會(huì)發(fā)生癌變，單純的網(wǎng)紋、發(fā)生在頰部、牙齦的扁平苔蘚，很少會(huì)發(fā)生癌變。
主要癌變的部位也是特殊部位，比如發(fā)生在舌緣的扁平苔蘚，反復(fù)糜爛，容易發(fā)生癌變。所以會(huì)要求扁平苔蘚的病人不管有沒(méi)有癥狀，應(yīng)該定期三個(gè)月至半年到醫(yī)院就診，當(dāng)扁平苔蘚出現(xiàn)糜爛和疼痛的時(shí)候要及時(shí)就診。
